These awards are a way to honour aged care staff who go above and beyond to achieve excellent outcomes.

We know our staff regularly go that extra mile to achieve amazing outcomes which may not be acknowledged so this is the chance to showcase their outstanding achievements. All you have to do is provide a reason for the nomination which include the outcomes of their exceptional work, attitude and/or innovation.

Clinical Excellence Award
Winner and Merit
Criteria:
Promote the mission, vision, and values of the Aged Care Facility.
Evaluate and manage clinical aspects of Aged Care Facility.
A commitment to safety and quality demonstrated through continuous quality improvement.
Maintains positive relationships with Clinicians, co-workers, administration, and residents.
Patient centric approach to care - commitment to ensuring the levels of service are always delivered to residents at all times.

PRESIDENTIAL AWARD (FACILITY OF THE YEAR)
Winner and Merit 
Nominations accepted from Hello Home Doctor Service Doctors, Chaperones, Call Centre and Administration staff. 
Criteria:
Embracing innovation to support after hours care.
Promotes collaboration and support for visiting doctors.
A commitment to quality clinical care.
Committed for continuing excellent care in after hours period.
Prompt and collaborative approach and work together for better care with the team at Hello Home Doctor Service.

NOMINATION INFORMATION 
1.  Nominations will be reviewed by a judging panel consisting of four Facility/Clinical Managers, two representatives from Hello Home Doctor Service representatives, and an independent person. 
2.  Nominations must be made by completing the Nomination Form and submitted by close of business 12th March 2021.
3.  Award recipients will be notified one week prior to arrange attendance at the 2021 After Hours Annual Forum.
