As you complete this form, you will be asked to provide information about yourself, the nominee and how the nominee meets the respective award criteria. Please do not start this nomination form until you have the following information prepared:
  • Current contact information for the nominee or support staff.
  • Narrative outlining how the nominee meets specific criteria (minimum of 450 words).
  • Optional - letters of support (maximum of three) from individuals outside your organization. *Program brochures, video clips, DVDs or published work will NOT be accepted.
