PURPOSE
To recognize a diabetes care and education specialist who has made a special contribution to diabetes care and education through dedication, innovation and sensitivity in person- centered care.  The Diabetes Care and Education Specialist will act as an "Ambassador" for  ADCES and should be someone who is considered a role model by their peers and colleagues. 
 
The winner may select either the current five face to face educational sessions in which he/she is asked to travel to Coordinating Body or Local Networking Group meetings and present an educational session or selecting the option to present 1-2 webinars and one session at the Annual Meeting in the year immediately following his/her selection.

Nominations may be submitted by a current ADCES Member or a State Coordinating Body.

The winner will receive an award of $5000.
 
ELIGIBILITY
Applicant/nominee must be, for at least 5 years of continuous membership at the time of application, an active member of ADCES as defined by the Association's Bylaws, including at the time of submission and at the time of receipt of the award.  Current and past members of the Board of Directors and past recipients are not eligible.

At the time of the application submission, the candidate must be a Certified Diabetes Educator (CDE) or BC-ADM  for at least 5 years.
 
APPLICATION PROCESS
Applications and supporting materials must be submitted by March 2, 2020.
  • All applications must be complete to be considered.  Please be sure to provide as much detail as possible in your answers.  Applications demonstrating thoughtful, detailed responses tend to yield more favorable results. 
  • If a confirmation has not been received by March 23, it is the applicant’s responsibility to contact ADCES at awards@aadenet.org.
  • Please include a curriculum vitae/resume and three letters of support as they relate to the nominee's ADCES (formerly known as AADE) activity.
