The "CENTRE" refers to all staff from Pathways Health Centre for Children, whether involved directly with your child or not. In addition to health care people they may include support staff such as office staff, custodians, administrative personnel, etc.
 
For each question, please think about how much the event or situation happens to you. You are asked to respond by selecting from one of the choices (To a Very Great Extent down to Not at All) that you feel best fits your experience.
