Progressive Life Center, Inc. (PLC) is making an effort to continuously improve the quality of the services we provide. The purpose of this survey is to get feedback from you regarding administrative services and the overall Agency. These surveys are also very important because they are used during the Council on Accreditation (COA) renewal process. Please take a moment to complete the survey. As mentioned above, all responses are anonymous, therefore, nobody, including the people reviewing the responses will be able to see who completed it. Because of this, we will not know who submitted a survey and who did not.
