Thank you for taking the time to submit the information of a deserving individual, family or organization in consideration for the 2019 USTA Wyoming Awards. Your help in honoring the deserving individuals and organizations that are helping to grow tennis in Wyoming is greatly appreciated. Before you get started, here are a few tips to facilitate the process:
1. Before nominating anyone for an award, please ensure you read the guidelines, criteria and overview for the award you are nominating them for. It is highly suggested that you prepare a list of accomplishments, activities and characteristics before starting the nomination process. Category descriptions are listed on the USTA Wyoming website as well as described briefly below.
2. The 2019 USTA Wyoming Awards are granted for "Grow The Game" activities that took place from late 2018 thru 2019...please ensure the supporting information & materials in your nomination form reflect these dates.
3. Note that this is not a ballot. The number of nominations a person receives has no impact on their being chosen as an award winner. For this reason, please nominate this person or organization only once per category.
4. Only one nomination can be submitted at a time. If you'd like to submit a nomination for another category, please complete this nomination form & create another for the additional category.
5. Please follow the directions carefully. Nominations that do not follow the directions will be at a distinct disadvantage and may not be included for consideration altogether. Nominations & supporting materials are due no later than Monday, September 16th
