Application Instructions for 2020 Strategic Initiative Award
PURPOSE
This award recognizes a member whose work reinforces ADCES 's (formerly known as AADE) current strategic goals to achieve the outcomes inspired by ADCES's Project Vision.
AWARD
Award recipient will be recognized at the ADCES Annual Conference with a plaque, receive a complementary registration to the Annual Conference, one-year Active Membership, and will be recognized in ADCES publication(s).
Candidates for the Strategic Initiative Award may be nominated by a peer/colleague or by self-nomination. Candidate must be an ADCES member who is in an active practice of diabetes self-management education and support (DSMES) with no conflict of interest.

ELIGIBILITY
Applicant/nominee must be an active member of ADCES (formerly known as AADE) as defined by the Association's bylaws, for at least 3 continuous years at the time of submission and receipt of the award. Past recipients are not eligible to apply for this award.
APPLICATION PROCESS
Applications and supporting materials must be submitted by March 2, 2020.
All Applications must be complete to be considered.
