Objective:
Provide nurses and staff an opportunity to give specific feedback regarding experiences with instructors and students of affiliate schools/programs who have been on their department for clinical educational purposes. Responses will aid in identifying positive practices that can be further promoted as well as opportunities for improvement.

1. State your department name
2. Select the school / program.
