Application Instructions for 2020 Rising Star Award

PURPOSE
ADCES (formerly known as AADE) seeks to honor a diabetes care and education specialist who has less than five years of diabetes care and education experience and has demonstrated leadership and commitment in his/her practice.

AWARD
Award recipient will be recognized at the ADCES Annual Meeting with a plaque, receive a complementary registration to the Annual Meeting, one-year Active Membership, and will be recognized in ADCES publication(s). 

CRITERIA
Candidates for the Rising Star Award may be nominated by a peer/colleague or by self-nomination.  Candidates must be an ADCES member who is in active practice of diabetes self-management education and support (DSMES) with no conflict of interest.

An appropriate candidate meets the needs of patients by using effective practices which may include:

- Innovation and use of new approaches to DSMES
- Working creatively in applying established scientific knowledge
- Demonstrates leadership skills and/or committed service to the practice
- Fosters community connections

ELIGIBILITY
Nominee/Applicant must be an active member of ADCES, as defined by the Association's Bylaws, including at the time of submission and at the time of receipt of the award with less than five years of diabetes care and education experience and has demonstrated leadership and commitment in his/her practice.
 
APPLICATION PROCESS
Award Candidate may be nominated by a peer or colleague, or may choose to complete the application on their own (self-nominate).

The nominator/applicant must address in writing how the nominee meets the criteria stated above in 1,000 words or less (essay).

The nominator/applicant must submit:
 
Two letters of support, of not more than 500 words, from a professional colleague other than the nominator.

CV or Resume
 
Applications must be submitted by March 2, 2020.

All applications must be complete to be considered.  Please be sure to provide as much detail as possible in your answers.  Applications demonstrating thoughtful, detailed responses tend to yield more favorable results.

Applicants should be an early careerist, with less than five years in the field of diabetes care and education.

An email confirmation will be sent after the completed application is received.  If a confirmation has not been received by March 23, it is the applicant’s responsibility to contact ADCES at awards@aadenet.org.
