2019 Megan's Wings Adopt-A-Family Donor/Volunteer Application

Thank you for helping support Megan's Wings adopted families with holiday needs and wishes this season! Megan's Wings is once again partnering with our wonderful donors and volunteers to bring the Adopt-A-Family Program to our pediatric oncology families in the Southern California region, who have financial and/or emotional struggles due to their child's illness. With your help, we hope to provide holiday joy while relieving some of the financial burdens of the holidays for these families.

Tax Applicability:
All donations made to Megan's Wings Adopt-A-Family Program, are tax deductible to the extent allowed by law. Megan's Wings is a 501(c)3 non-profit organization. Federal Tax ID #56-2501762.

Option A - Full Adoption:
Kids Gifts: I will purchase and wrap gift/s for each child in the family from their wishlist. (spending min $50/child). Gift Cards: A - I will purchase gift cards for the family. ($100 gas, $200 grocery and $100 Kohl's) B - I prefer to make a $400 tax-deductible donation to Megan's Wings to cover the cost of gift cards, and MW's will purchase the GC's thru corporate and let the family know you adopted them. You'll receive 100% tax deduction for your donation to Megan's Wings, a 501c3 charity. (Buying thru corporate is more secure than buying from the grocery store rack. Unfortunately, we've had fraud from store-bought GC's so we recommend this option.) Holiday Food Bag: A - MW will provide you a shopping list to buy groceries for a holiday meal (dry goods only). B - if you prefer, we can provide the food from our holiday food drives. Care Packages due Dec 5: Deliver your family's completed care package to MW's home office, 1735 N Kelly Ave, Upland between Dec 2-5. Drop off time: 4:30-6:30 pm or call to make appt. (909) 532-8135. If you'd like to try and meet your adopted family, sign up to help at a Holiday Event (Option G) where families pick up their care package. If you choose to make the $400 gift card donation, bring a check when you drop off your gifts or go on our website - www.meganswings.org & make the donation from our Donate Now button on the homepage. Option B - Partial Adoption:
You agree to purchase gifts from wishlist for each child in the family spending a min $50/child. Megan's Wings will provide the gift cards and food basket to complete the care package.

Option C - Be A Monetary Sponsor:
Don't have free time, but really want to Adopt-A-Family? Make a tax-deductible monetary donation to Megan's Wings! A MW volunteer elf will shop, wrap and prepare your family's care package for you, and we'll let the family know you adopted them! A $500 monetary donation covers the cost of full adoption for a family with 2 children. $200 covers the kid's gifts for a family with 4 children. All donations big or small make a difference. Option D - Sponsor a Family At Megan's Home:
Make a tax-deductible year end donation to help house a family at Megan's Home, providing temporary housing for families needing to stay close to the hospital. A $150 donation covers the costs for 1 week, and a $600 donation sponsors a family for 1 full month at Megan's Home. Option E - Volunteer Shop for Kids Gifts:
Please send me a family wishlist, and I will shop, wrap and deliver wrapped gifts back to MW's home office, 1735 N Kelly Ave, Upland between Dec 2-5. Drop off time: 4:30-6:30pm or call to make appt. (909) 532-8135. I agree to stick to the budgeted $50/child, and be reimbursed from Megan's Wings after turning in my receipts.

Option F - Volunteer as Santa's Elf:
I'd love to help bring joy to kids in the hospital at Kaiser Fontana and Loma Linda Hospitals on Friday, Dec 13th from 8:00am - 4pm. Help bring toys and family care packages to kids in the hospital.

Option G - Volunteer at a Holiday Event:
I'd love to help at a holiday event on Dec 7 in Hesperia or Dec 8 in Ontario from 10:30am - 1:30pm where families will pick up their care package. In comment section, let us know what day you are available, and we'll call you with details.

Option H - Host a Teen GC Drive:
We need teen gift cards like jamba juice, starbucks, Target, etc for our Dec 13th Santa visits in the hospitals!

Can spend a minimum of $50 per child. Use best judgement on age appropriate gifts. Call if you have any questions.

Provide children in the same family with the same number of gifts. If there's a different # of gifts for the kids, wrap them together to equal the same number for each child.
We all know kids keep track of how many gifts their siblings get! Please help us make sure it's equal!

Wrap gifts and tag with FIRST name only. Please DON'T put who the gift is from.
Parents sometimes use these gifts from Santa or themselves.

Place all gifts in a large plastic black bag or Christmas Bag with the family name taped on the outside of the bag. If you have a large family and gifts can't fit in one bag, label (1 of 2, 2 of 2). The Dollar Store has festive large plastic bags for $1.00!
