2020 Lifetime Achievement Award

PURPOSE
This award recognizes outstanding contributions and service across the diabetes landscape. This award honors an individual who serves as an extraordinary role model and has dedicated themselves to diabetes care be that through research, care, self-management or on-going support.  This award is intended to be broad in its scope including but also moving beyond service to the Association of Diabetes Care & Education Specialists ( formerly known as AADE) and its members.

AWARD

The award recipient will receive a complementary registration to the Association of Diabetes Care & Education Specialists (ADCES) Annual Meeting, where they will be recognized and a $1,000 travel stipend. The award recipient will also be given a Lifetime membership.

Applicant/nominee does not need to be an ADCES member. The candidate must be active in the diabetes care and education field for a minimum of 20 years.  Past recipients of this award are not eligible. Current and incoming members of the Board of Directors are not eligible.

Applications and supporting materials must be submitted by March 2, 2020.
All Applications must be complete to be considered.
