2019 K-CountUnsheltered Survey for KY Balance of State Continuum of Care Point-in-Time Count

Welcome to the 2019 K-Count UNSHELTERED Survey Data Entry Page.

You have entered the K-Count survey for UNSHELTERED INDIVIDUALS AND FAMILIES (Households staying outside and other places not meant for regular human habitation).

If you are entering data collected from a paper copy of the survey and used the Sheltered Survey Form for persons staying in emergency shelters, transitional housing projects, or hotel/motel voucher programs (refer to the top of the paper survey form you are entering to check) please use the Sheltered Survey Data Entry Page. If you are entering data from the Unsheltered Observation Survey Form, please use the Unsheltered Observation Survey Data Entry Page.

Instructions

Stop! Before you begin entering responses from a paper survey form, please take a moment to review the information recorded to ensure questions have been answered. Most importantly, you must make sure you have the age range (see Q17), gender, race, and ethnicity (see Q18) for each member of the household included on the survey. The total number of persons in the household should be recorded on Q16.

If any of this demographic information is missing for a member of the household, you will not be able to submit this form. If missing data exists for these categories, we suggest you contact the person who conducted the survey and ask them if they recall the information. If the respondent refused to give a response, the surveyor should have recorded his/her (the interviewer's) best guess.
