Application Instructions ADCES Innovative Use of Technology Award

PURPOSE
The award supports the innovative use and integration of software and/or technology into practice. The project submitted should have significantly improved self-management among individual patient practices or broad diabetes populations. 

AWARD
Award recipient will receive a complementary registration to the Annual Meeting, where they will be recognized.
Recipient may also have the chance to share the idea/tool and any preliminary results with attendees through an educational poster or session (or both) at the Annual Meeting if deemed appropriate by the Annual Meeting Committee.

CRITERIA
Submissions for this award will be reviewed according to the following criteria:
  • Significance of the innovative use and integration of software and/or technology in enhancing practice.
  • Potential to affect large diabetes populations.
  • Clarity, feasibility, sustainability.
  • Potential for other educators to use project/tool in their practices.
  • Alignment with ADCES mission and strategic direction.

ELIGIBILITY
Applicant must be, for at least one year, an active member of ADCES, as defined by the Association's Bylaws, at the time of submission and at the time of receipt of the award.

Should there be co-developers, they should also be members of ADCES.

Applicant must have an affiliation with an organization that can accept and administer the funds on behalf of the recipient. We cannot release funds to an individual; the recipient will be asked to provide a current W-9 for the organization.

The proposed project must be completed within twelve months of funding to be eligible to present at the ADCES Annual Meeting and Exhibition the following year; the recipient will be required to submit an abstract by mid-October 2020 for the 2021 Meeting.

APPLICATION PROCESS
The applicant must provide a narrative description of the project, target audience, purpose and results. Application materials must include a completed Cover Page (page 3 of application) or they will be disqualified.
 
Your application should include responses to the following:

1.  What are the overall objectives as well as the specific teaching/learning objectives?

2.  Describe the audience and setting for which the technology and/or software was targeted. Include age range, ethnic group, literacy level, socioeconomic level, etc.

3.  Describe what outcome measures were used to evaluate effectiveness.

4.  State the anticipated impact/outcome of the implementation of the tool/program.



You will receive an email confirming your awards application by March 23, 2020. If you have not received an email confirmation, it is the applicant’s responsibility to contact ADCES at awards@aadenet.org.
