2019 IOWA SPIL - Disability Community Input Survey

Request for Input from Iowans With Disabilities and Others For SPIL Development

We would like to ask Iowans with disabilities, as well as family members or friends of someone with a disability and disability advocates/service providers, to please give us your input on the development of the new State Plan for Independent Living (SPIL).

The SPIL lays out the activities we will conduct to strengthen the six Iowa Centers for Independent Living, and what we will do to strengthen services and resources for people with disabilities so they can remain living independently and not be forced to live in an institution such as a nursing home.

THANK YOU for taking the time to give your input!

For each question, please Rank Highest Level of Importance for You Personally as a Person with a Disability, as a family member or friend of someone with a disability, or as other disability advocates/service providers.

You may go back and change your responses at any time up until you submit the survey.
