Autism Connections - 2019 Flutie Grant ENGLISH Application |
1. Application Guidelines
Autism Connections is happy to announce that the Doug Flutie, Jr. Foundation for Autism has once again funded a grant program for underserved low and moderate-income families living in Hampshire, Franklin, and Hampden Counties, as well as the Worcester County towns of Royalston, Athol, Petersham, and Phillipston. The $10,000 provides allocations to families of children with autism and may be used for a wide variety of purposes related to best supporting the individual.
All families who meet the financial and other criteria for the grant will be in contention to receive a $300 allocation. Qualifying entries will be entered into a lottery system and grants awarded. For families with multiple eligible children, please submit one application for each child.
Those applying for the grant must meet the following criteria:
• Must be a family of a child or adult child with a diagnosis on the autism spectrum residing at home
• Must be living in Hampshire, Franklin, Hampden Counties, or the Worcester County towns of Royalston, Athol, Petersham, and Phillipston
• Must NOT be receiving services through the MA Autism Waiver Program
• Must NOT be receiving services through the DDS DESE Program
• Must NOT be receiving Adult DDS Services
• Must NOT be receiving services through the DDS IFFS Program
• Must be willing to turn in receipts from funds used and submit a thank you note to the Doug Flutie, Jr. Foundation for Autism
ALL APPLICATIONS ARE DUE BY FRIDAY, May 15, 2019 AT 5:00 PM

Respite care
Educational conference fees
Skating lessons
Sports equipment
Art classes
Behavioral consultation
