Introduction for parents and guardians

BEFORE COMPLETING THIS REGISTRATION FORM it is imperative that you read all of the following information about the Youth Fishing Camp and respond to the RULES as set forth.

We will not accept any child to the Youth Fishing Camp who's parent/guardian has not agreed to the terms set forth.

The Youth Fishing Camp is conducted by the Rhode Island Saltwater Anglers Association and it's volunteer members.
THERE IS NO COST TO THE FAMILY OF ANY CHILD THAT IS ACCEPTED.
The total cost is paid with grants from the R.I. Saltwater Anglers Foundation, the R.I. DEM Aquatic Resources Education Division, the RI Saltwater Recreational Fishing License Fund and the US Fish & Wildlife Service Sport Fish Restoration Fund.

* Up to fifty (50) children will be accepted to the program.
* All children must be between the ages of 7 to 12 years old.  We reserve the right to request proof of age.
* Children will be placed into groups sorted by age and experience.

Priority will be given to children who have not attended the Fishing Camp in the past.

(Children who have attended in the past may sign up, but will not be accepted until the end of the registration period if there is available space)
* Only Rhode Island residents (full time) are eligible

YOU MUST COMPLETE THIS SURVEY FOR EACH CHILD YOU ARE REGISTERING.

Following is a series of YES/NO questions.   If you are unable to agree to them, please do not proceed with the registration process.

* 1. My child can attend all three days from 8:30 AM to 3:30 PM.

* 2. On Day 1 (Tuesday), all children will have shore-based instruction from RI DEM staff, Coast Guard Auxiliary and RISAA members, plus learning to cast from shore.

* 3. On Day 2 (Wednesday) all children will be transported by charter bus from Rocky Point Park to Pt. Judith where they will board the 80 foot fishing boat, Seven B's V for several hours of fishing. While on board they will receive a free hot dog or hamburger, chips and bottle of water. They will return in time for pick up at 3:30 PM.

* 4. On Day 3 (Thursday) all children will be transported by charter bus from Rocky Point Park to Brewer Greenwich Bay Marina (5 miles) where they will board RISAA members' boats and fish for several hours in local waters near Rocky Point and Greenwich Bay.

* 5. On Tuesday and Thursday the children will provided with a buffet lunch prepared by a professional caterer.  It will consist of chicken fingers, pizza squares, hot dogs, macaroni & cheese, cookies, brownies and bottled water.  Is this acceptable?

* 6. Photos and videos will be taken during the three day Youth Fishing Camp, by our own photographer and videographer and possible area TV and newspapers.   Is this acceptable if your child appears in a photo or video?

* 7. I understand that if my child misbehaves, become disruptive or fails to cooperate with instructors or chaperones, I will be contacted and agree to immediately respond to pick up my child who will NOT be allowed to return.

* 8. Children will NOT be allowed to have a cell phone during camp. The camp director will have a list of all parent/guardian contact information in case of emergency. If parent/guardian sends a child to camp with a cell, it must be left with the camp director on arrival in the morning and will be returned to child at end of day.

* 9. No child will be allowed to possess/carry any pills on their person while at Camp.  If medication may be required during the day, it must be left with camp director and/or the on-site first aid/EMT person.
This also includes sunscreen or any other type of lotion or chemical.

* 10. Parents will be required to sign a Release of Liability form prior to any child taking part in the Fishing Camp.  This will be sent to parents on notice of a child's acceptance.
