Application Instructions for FELLOWS Application
The Fellows Program of the Association of Diabetes Care & Education Specialists was established to recognize diabetes care and education specialist leaders who have made outstanding contributions to ADCES through volunteer work at the local, state and national levels, diabetes care and education through clinical practice, research, education, or health policy, including the development and implementation of mentorship programs for diabetes care and education specialists.
Individuals who desire to be considered for Fellows selection must meet the following minimum requirements:
• Possess a Baccalaureate degree
• Be a current ADCES member with a minimum of 5 consecutive years of membership
• Have 10 years of experience as of January 1st of the current year in the field of diabetes care and education as an educator, clinician or researcher
▪ Be a leader who has made outstanding contributions to ADCES and to diabetes care and education, through clinical practice, research, education, or health policy. This includes development and implementation of mentorship program for diabetes care and education specialists.
• Be involved in ADCES at the Local and National levels
In addition to meeting the minimum requirements, candidates must include with this application:
• A Personal Essay (limited to 750 words) that includes contributions to ADCES and diabetes care and education
• Two Letters of Recommendation (each limited to 500 words), one which should be from a current ADCES Fellow
• A non-refundable, non-transferable application fee of $250
Only complete applications received by the April 1, 2021 deadline will be considered. All applications must be submitted electronically. The Fellows Review Committee will review all applications. Those selected to become 2021 Fellows will be notified within 90 days of the deadline.
