Thank you for visiting the 100 Distinguished Public Health Nurses in NC nomination site and completing a nomination for a Distinguished NC Public Health Nurse.

FOR BEST RESULTS: First compile your answers in a Word document to assure they meet the required maximum 500-word count limit, then copy/paste your answers into this survey. 

All nominees must be practicing at the time of nomination and expected to be practicing in Fall 2019 when the awards are distributed.  
 
Helpful Hints for completing this nomination:
  1. The nomination must be completed once the survey is started - you cannot save your work and return at a different time.
  2. It is recommended that nominators compile their answers in a Word document and assure they meet the required word count beforehand, then copy/paste answers into this survey.
  3. Nominators must select the specific professional criteria category under which the public health nurse they are nominating should be recognized. 
  4. The nominator must then supply responses to each Standard for which they want to nominate the public health nurse.
  5. All information must be submitted, and all questions answered, for a nomination to be considered.  
  6. A picture/headshot must be uploaded into this survey and is required to be submitted if the nomination is to be considered. The photo must be in JPEG, JPG, PNG, or GIF format and MUST include the nominee's FULL NAME in the file name.
    NOTE: Only photo files up to 16 MB can be loaded.
  7. If a person is nominated for more than one Standard, the average score for all responses will be used.
  8. Because of limitations of the SurveyMonkey platform, you may not be able to see your entire answer to each Standard you are nominating for.  Therefore, it is important that you verify that your answer is complete in your Word version before copying/pasting your response(s) into this form.
  9. You must click "Submit My Nomination" at the end of the survey to complete the nomination process.
The 100 Distinguished PHN in NC Award Committee members are not eligible for this award.
