Dear HIV Prevention and Care Colleague: 

AUDIENCE:  You are receiving this survey request because you are a valuable part of Connecticut's HIV prevention and care workforce - as a member of a healthcare team and/or as an administrator. 

PURPOSE.  Federal agencies that fund HIV prevention and care service in Connecticut require that the state survey its HIV prevention and care workforce.  

CALL TO ACTION.  Please take 12 to 16 minutes to complete this ANONYMOUS survey developed by a statewide group of HIV Funders and the Connecticut HIV Planning Consortium.   The data collection period will close on or about December 15, 2019. 

TOPICS. This survey asks questions about your demographics, knowledge, attitudes, and beliefs relevant to HIV prevention and care.  

ANONYMOUS.  The survey does NOT ask you to share any personally identifiable information such as your date of birth, name, or organization name.  The survey results will be analyzed at the county-wide and statewide levels.   

HIPAA COMPLIANT.  This method of data collection meets HIPAA standards for data security.   

RESULTS.  The HIV Funders Group and the Connecticut HIV Planning Consortium (CHPC) will use these results to increase access to professional development and training opportunities that will better support Connecticut's HIV prevention and care workforce.
