Please fill out the nomination form as completely as possible. Note that resumes and CVs will not be accepted and cannot be submitted in place of the nomination form.

Each nomination form must be accompanied by a letter of recommendation for the nominee from a past or current UAH faculty or staff member OR a past or current member of the UAH Alumni Association Board of Directors. (Nominators who are UAH faculty or staff may write and submit the letter of recommendation themselves if they choose.)

Submitting a nomination does not guarantee selection as an award honoree. 
 
Please do not notify your nominee of his/her nomination. All nominations are asked to be kept confidential.

Honorees will be announced following selection and confirmation by the UAH Alumni Association Board of Directors. Awards will be presented at the Alumni of Achievement Awards in fall of 2019 (date and details to be announced).
