2018 Summit Shuttle Form

Applicant Information

This form must be filled out by parents/guardians of participants under the age of 18, or adult participants (18+). If you have already filled out the form and wish to revisit it or make a change, please contact Sarah Parker at sarah_parker@ncascades.org or (360) 854-2579 (Day of Summit: 978-347-2186).

Please have the Summit Shuttle Schedule available while filling out this form. The schedule was emailed to you with the Welcome Packet and is listed on our website. 

1.Participant name(Required.)
2.This form must be filled out by an adult participant (18+) or the parent/guardian of a participant under 18.(Required.)
3.Please indicate which Summit Shuttle stop you/your child would like to be picked up at.(Required.)
4.Will you/your child take the Summit Shuttle to the event in the morning and back in the evening? If you plan to take the shuttle one way, please expand here.(Required.)
5.For parents and guardians of minors: If you are late in picking up your child, do you give permission for the Summit Shuttle to move on to the next shuttle stop and for your child to wait by themselves?(Required.)
6.For parents and guardians of minors: in the case that you are not available to pick your child up at the shuttle stop, do you give permission for your child to take public transportation, walk, or catch a ride home with someone else?(Required.)
