Dear MEDIA rep and/or Freelance Photographer...

This form has been created to gather information needed for each photographer and videographer who requests a MEDIA vest for infield access. USA BMX gladly welcomes the Press to the infield of any USA BMX national event. Working Media is always our highest priority.
We do require you fill out the a form at the National event, which will require Credit Card information as a "deposit."  Your credit card WILL NOT be charged as long as you return the vest at the end of the day. Your deposit slip will be given back to you when the bib is returned.
If you fail to return a vest, your card will be charged $50.

All MEDIA vests must be returned at the end of weekend, and worn at all times when you are in the infield.
Failure to wear your Media vest may result in you asking to leave the infield area of the track.

Thank you for taking the time to provide us with your information. We look forward to seeing you at the races!

* USA BMX will request a copy of all photos/videos shot by QuadCopters or Drones at our events - to be used for future promotional materials and possible use in PULL magazine.
