Copy of 2017-2018 Survey for Stakeholders

The purpose of this survey is to assess the effectiveness of the Council’s advocacy, capacity building and systems change activities and assist the Council in future planning.  The federal Administration on Developmental Disabilities (ADD), which funds the Council, requires that the Council use these questions to determine the systemic impact of its activities through the responses of stakeholders including advocacy groups, service providers, government agencies, policymakers and community organizations.
