In order for us to continue securing funding for our programs, we need to show the long-term impact. Your answers will be kept confidential unless you list (at the bottom) that we can use your story - we are only reporting out the summary of all findings.
As a bonus for completing this, you will be entered to win $500 (1 prize), $200 (1 prize), or $100 (3 prizes available).

Also, once you take it and refer other Winning Futures Alumni to take this survey you will be entered into an additional drawing. We will draw five winners of $50 each!
