2018 Salary & Fringe Benefit Survey Purpose of this form: To notify the Michigan Federation for Children and Families of our agency's intent to participate in the 2018 Salary and Fringe Benefit Survey. Once the 2018 survey is open for data entry, on or about January 31, 2018, the survey instrument will be made available to the contact person identified below. The survey will be available in hard copy (pdf) for handwritten purposes and as an Excel spreadsheet; you may choose either option. Completed 2018 surveys are due by February 28, 2018. Submission can be made by fax, email or mail service. I would like to purchase a copy of the 2016 Salary & Fringe Benefit Report for $150. Report can be mailed to you immediately upon payment.
