Noojmowin Teg Health Centre is undergoing an external evaluation of its programs and services and we invite your feedback!

Your responses will only be seen by the external consultants and will be kept confidential. A summary of the responses from all participants will be prepared for Noojmowin Teg Health Centre and be included in the final evaluation report. We will also report the number of communities and organizations that have participated in this survey.

Your name or organization will NOT be linked to the answers you provide and program staff will NOT be able to connect your responses to your name. External consultants will separate all identifying information from the evaluation feedback. Your privacy will be respected.
