The Consideration to Decline Form has been developed to allow a District Referee Coordinator or Club Head Referee to provide information to Ontario Soccer in regards to a Match Official that has been Auto-Moved to the next grade, and there are legitimate concerns.

This form will be reviewed by Ontario Soccer, and forwarded to the Match Official and the Club Head Referee and/or District Referee Coordinator.

Considerations to decline could include, but are not limited to:

Returning from injury – physically unable
Official discipline records
Reported and tracked Code of Conduct breaches
Tracked history of declines, no-shows, or late arrivals
Insufficient activity to merit a promotion
