PWN Member Survey Dec 2018 |
Basic information
This survey is for PWN members. We define members as any women and people of trans experience living with HIV who choose to identify as members of Positive Women's Network-USA.
We are collecting demographic information on respondents. We understand that this information is personal. In order to protect confidentiality, we are not collecting names or contact information as a part of this survey.
At the end of the survey, once you complete it, you will be offered the opportunity to go to a separate short form to enter your contact information in order to be entered in a raffle to win a $100 gift card. That contact information is NOT linked with your responses to this survey.
The PWN survey will take less than 30 minutes to complete. Please plan to complete it in one sitting when you have privacy to answer questions honestly. The survey will be open until 11:59pm PST on Wednesday, December 5, 2018.
