2017 Survey

Dear Fellow,

The primary Core Values of the USA Section of the International College of Dentists are Leadership, Service, and Integrity. These are the key determinants that result in the invitation for Fellowship. All Fellows exhibit significant contributions that have benefited mankind or the dental profession.

It is very important that the USA Section of the International College of Dentists has knowledge of the types and numbers of humanitarian projects that the USA Fellows participate in. This information is critical to enhancing our potential corporate relationships as well as disseminating it within the ICD family, thereby encouraging other Fellows to become involved.

The USA Section’s Foundation asks that you respond to the following survey in order for us to better identify and support the local or state humanitarian projects of USA ICD Fellows.

Thank you in advance for participating in our survey.
