Application Process

Eligibility:
-Families must fit the illness/crisis criteria listed stated in the application.
-Families must have children between the ages of 6 weeks-18 years old living in the household.  
-Family members attending must be living in the household of the applicant.
-Families must commit to attending the entire weekend from Friday at 11:30am until Sunday at 1:30pm. 
-Families must be able to provide their own transportation to and from the retreat.  
-Families must participate in all scheduled activities.
-Families can not attend more than once.
 
 
After you have submitted the online application you must fax, scan or mail a letter from a healthcare professional stating the applicants current diagnosis including any physical,  emotional or cognitive delays, medications and allergies.  (Excluding Post 9/11 combat veteran retreat and death of a loved one retreat).
 
 
Families applying for the retreat for post 9/11 combat veterans are required to submit a copy of their veteran's DD Form 214 (Discharge Papers and Separation Documents).
