University of New Mexico Informed Consent for Survey Participation

The NSF funded Southwest Center for Microsystems Education (SCME) from the University of New Mexico is conducting a research study to determine the value of professional development activities, use of SCME educational materials, and impact on students. You are being asked to participate in this study because you have engaged with SCME and/or SCME educational materials.

Your participation will involve responding to this survey in order to gauge effectiveness of our work and determine how SCME can better assist you in implementation of our educational materials in your classroom. The survey should take about 10 minutes to complete. Your involvement in the study is voluntary, and you may choose not to participate. You can refuse to answer any of the questions at any time.

The findings from this project will provide information on SCME’s effectiveness and identify improvement opportunities. If published, results will be presented in summary form only.

If you have any questions about this research project, please feel free to contact Dr. Matthias Pleil (mpleil@unm.edu) or Barbara C. Lopez (botero@unm.edu). If you have questions regarding your legal rights as a research subject, you may call the UNM Office of the Institutional Review Board (OIRB) at (505) 277-2644.

By completing this survey, you will be agreeing to participate in the above described research study.

Your participation is valued and very much appreciated!

Kind Regards,
