Self-Represented Litigant Intake Form

The NSRLP is committed to continuously populating our database of self-represented litigants (SRLs) with background information and demographic data collected with consent. If you are willing to contribute your information to our database, we kindly ask that you take a few minutes to complete the following survey. (The survey usually takes about 15 minutes to complete, though may take longer if you wish to tell us more.)

The survey asks specific questions about your experience as an SRL, and many questions are a simple yes/no. At the end of the survey, we ask that you share in your own words how self-representation has impacted your life, and offer any tips you may have for other SRLs. Your answers are placed into a secure database. The information you provide will be used in the following ways:

1. To develop further data about SRL demographics and experiences;
2. To match SRLs to organizations and agencies requesting a speaker or a working group member, or similar participation by a SRL, with permission;
3. To provide participants for user feedback projects conducted in courts and tribunals;
4. To generate quantitative data to further inform the discussion about the SRL phenomenon and access to justice issues in Canada.

Furthermore, the data collected is used to publish regular reports which highlight the trends associated with self representation. Thank you for taking the time to provide this information about yourself and your experience as a self-represented litigant.
