Self-Represented Litigant Intake Form
We are not currently conducting full research interviews; however, we are continuing to populate our database of self-represented litigants (SRLs) with background information and demographic data collected with consent. If you are willing to contribute your information to our database, we kindly ask that you take a few minutes to complete the following survey.
The survey asks specific questions about your experience as an SRL and most questions are a simple yes/no. For the last question in the survey, we ask that you share in your own words how self-representation has impacted your life, and offer any tips you may have for other SRLs. Your answers are placed into a secure database. The information you provide will be used in the following ways:
1. To develop further data about SRL demographics and experiences;
2. To match SRLs to organizations and agencies requesting a speaker or a working group member, or similar participation by a SRL, with permission;

Furthermore, the data collected is used to publish regular reports which highlights the trends associated with self representation. Thank you for taking a few moments to provide this information about yourself and your experience as a self-represented litigant.
