2016 Cumberland County Community Health Needs Assessment

The Cumberland County Department of Public Health and Cape Fear Valley Hospital are collaborating with several other community agencies to conduct a survey of our county. The purpose of the survey is to learn more about the health and quality of life in Cumberland County. The survey results will be used to help address the major health and community issues in our county. A community health assessment report and action plans will be developed based on the survey data and additional data pulled from state databases.

The survey is completely voluntary, and it should take no longer than 20 minutes to complete.  Your answers will be completely confidential. The information you give us will not be linked to you in any way.
