1. General Information

The following questions are aligned with the SLV Charter LCAP goals & strategic plan and have been revised by our Parent Advisory Committee. Your responses will provide important feedback to support the ongoing improvement of the Charter School and all its programs. There are parts of the survey that are designed for all programs, and some designed for specific grade levels or programs. Please complete the parts that are appropriate for your child's involvement in the Charter.  We consider your feedback very important, and use this data to assess potential changes for next year.  

NOTE: If you have more than one child in more than one program, I have added a final page where I have duplicated the questions that you may have a different answer for.  We are hoping this will allow you to think of how the Charter supports each child, separately.
