2011 Chameleon's Journey Parent/Guardian Survey

3.Parent/Guardian Evaluation

It was a pleasure to have you participate in the 12th Annual Chameleon’s Journey overnight grief camp. It is our goal to provide the best experience possible in supporting you and your children through the grief process. Each year we provide an opportunity for you to share your thoughts regarding the grief camp. The feedback allows us to continue to make adjustments to improve the camp. Please let us hear from you by taking a moment to complete this brief survey.

Thank you.
