2019 Affiliate Event Application

If you would like to request space for an affiliate event during the 2019 ACR/ARP Annual Meeting, November 8 - 13, 2019, please complete this application. Upon review by the ACR, you will receive an email confirmation that your event has been approved. Please allow 5-7 business days for a response. The approval email should be forwarded to your contact at the hotel you choose so they know your event has been approved.

I have read and agree to abide by the affiliate event Approved Times, Policies and Categories including the Affiliate Events Schedule.

I have read and agree to abide by the Rules and Regulations.

I understand that if my events are not in compliance with ACR policies, I will be contacted by ACR to change the event so that it is in compliance or I may incur penalties and/or the event may be cancelled.

I understand the ACR is not responsible for contractual agreements by the event organizer or any costs associated with or incurred by affiliate events. In the event of cancellation, I understand that the event organizer or sponsor will be responsible for any venue costs or cancellation fees.

I understand that offering CME during an affiliate event is prohibited.
