2017 Pearson Accessible Math User Study: Recruitment Questionnaire for Students 18 and Over

The study will be held at the National Federation of the Blind 2017 National Convention

Please complete the following items. This will help us determine if you are a good match for this study.

2.Have you completed Algebra 1 in high school or middle school?
3.Do you have an identified or documented visual impairment? (This could include a designation in a Section 504 Plan, an IEP under IDEA, or other primary diagnosis as having a visual impairment, including such conditions as blindness or low vision).
4.How would you best describe your visual impairment?
5.Are you an English Language Learner (ELL or ESL)?
6.Do you have any of the following conditions: severe hearing impairment, cognitive or intellectual disability, or a severe motor impairment?
7.If you are a braille reader, do you use Nemeth braille for math and/or science coursework?
8.Do you use a refreshable braille display together with a screen reader (for example, JAWS, NVDA, Window-Eyes or VoiceOver) to access on-line content?
9.Are you comfortable sharing your opinion while being recorded?
