Consent & Liability Waiver
I hereby grant permission for my child to participate in various athletics, programs, events, trips or activities (hereinafter referred to as “Activities”) operated or sponsored by Lutz Preparatory School, Inc. (LP). 

I understand that participation in Activities may involve injury of some type to either my child or another student.  Such injury can include direct physical injury to one’s body and the possibility of emotional injury experienced as a result of witnessing or accidentally inflicting injury to another.  The severity of such injury can range from a minor injury to a more serious injury or even death.  Risks that may be encountered may include cardiac or respiratory distress; bone or joint injuries; back or muscle injury; slipping, tripping or falling; lifting, twisting, straining, spraining or breaking an appendage; or other trauma or injury.  Injuries may result from the correct or incorrect performance of techniques or equipment used in Activities, from ill-fitting or worn equipment, from the administration of first aid, from failing to follow training, safety or other rules.  This list is non-exhaustive and is provided by way of illustration of the risks or injuries that may be incurred.  By signing this agreement, I understand and assume all risks inherent in these Activities and accept full and complete responsibility for any and all damages or injuries of any kind; but I am also aware that reasonable care and supervision will be exercised by the adult supervision to provide for the general well-being of my child.  I accept responsibility for notifying LP coaching staff of any pre-existing medical conditions affecting my child as well as providing a sports physical that has been completed by a licensed medical professional.

I, for myself, my heirs, successors and assigns do hereby release, discharge and waive any and all responsibility of Lutz Preparatory School, Inc., its officers, directors, agents, employees, volunteers, and members from and against all claims of liability for any and all trauma, injury, damage, expense, handicap, disability (including death) and for damage to or loss of property which may be suffered by myself or my child arising out of, or in any way  resulting from or attributable in whole or in part to the traveling to or from, using any equipment of, performing maintenance for, or participating in LP Activities.  I also agree to release, hold harmless and indemnify LP, its officers, directors, agents, employees, volunteers and members from and against all liability, claims, demands, actions, loss, costs, fees, and damage, even if arising from their own negligence, arising out of my or my child’s participation in LP Activities.  I knowingly and voluntarily assume the risk and liability of any damage or injury, regardless of severity that I may incur to myself or my child or others as a result of participation in LP Activities.
I have read and understood LP’s rules pertaining to LP Activities and agree to abide by them and adhere to standards of appropriate conduct at all times.  I agree to procure or am already covered by insurance adequate to cover any claim resulting from participation in LP Activities. I accept full responsibility for all medical expenses and claims incurred as a result of my participation. I authorize the Athletic Director, School Director, Coach or Sponsor in attendance at any LP Activity to select and secure medical attention, at my sole expense, as may be necessary for my child as a result of injuries or other events requiring emergency care while I/we are not in attendance at such event. 

This release is binding on me and all other persons, including family.
Sportsmanship Contract
Lutz Preparatory desires to have a well-organized competitive league in an environment where the highest value is people. LP welcomes all individuals to participate and asks that you contribute to creating and maintaining this kind of environment by agreeing to this sportsmanship contract in the following manner: 

1)  Encourage members of both teams.
2)  Keep all comments positive.
3)  Only players and coaches allowed on field, no exceptions.
4)  No criticizing, suggesting strategy, or discussing playing time with any coach.
5)  Criticizing the officials, coaches, or opponents will not be tolerated. The same goes for abusive language or cursing.
6)  You are responsible for all family members and friends attending the games to watch your child.
7)  No open alcoholic beverages on fields or parking areas.

Please report any violations to the coach or administrator present at the game.  Please remember this is a middle school sports league and everyone makes mistakes.  Get involved to make this the best experience for all.

Enforcement:
1)  Individuals will be given one warning for inappropriate activity.
2)  Individuals that continue will be asked to leave the field and their child will be removed from the game until the spectator has left.
3)  Other sanctions as deemed necessary by the administrator or board may be enforced, including permanent ban.
4)  The second time a spectator is asked to leave a game they, and their child, will AUTOMATICALLY be banned for the rest of the year, for all sports.
5)  All decisions are final. There will be no hearing or appeals.
Concussion Information Sheet

Your child needs the sports physical, that is part of the general athletic “packet”, completed before trying out for their respective sport. Their well visit physical is NOT the same.
If your child has had a sports physical done within the 365 days, we will simply need a copy of the completed form at the beginning of EACH school year (previous year physicals will be archived each summer).  Make sure you keep a copy for your records in the event they are valid for multiple school years.
All items must be completed online by the deadline specified for their respective sport or your child will not be able to participate.
Your child is required to stay after all sporting events to help with cleanup including equipment, garbage, and any other items that need to be taken care of.
Your child needs to maintain a 2.5 QPA in core classes as well as no failing grades in order to be able to be eligible for participation. 
Once they drop below that average they are automatically suspended from the team until they raise it to the appropriate level.
If your student is absent from school for any length of time for an illness related reason, they are NOT eligible to play that day under any circumstance.
You are responsible for timely pickup of your child after practice and games (10 minutes). After the 10 minutes is over your child will be escorted to the Pre-prep aftercare and you will be responsible for any charges incurred.
The 3rd time there is an issue the coach and administration has the right to not let your child return to the team.
Your child is responsible for their behavior both on and off the court. If there is an issue during an athletic event or at school the coach and administration reserve the right to make your child ineligible for a designated period of time.
Once your child has joined the team, the enrollment fee or is non-refundable under any circumstance.
