1. Complete the form below for your chosen Provider(s). All requests for funding must be submitted to ASC on or before September 23, 2016 at 5:00pm. The form will close at that time. Please note: if any of your programs will begin in October, you should submit your Spending Plan as early in September as possible in order to allow for adequate processing time; do NOT wait until September 23rd.

2. Make sure you have contacted the Provider to determine pricing and availability prior to submitting your request.

3. ASC will pay the Provider(s) directly upon completion of the program and receipt of all evaluation forms. Schools should not pay funds directly to the Provider.

4. List CMS Transportation as an additional Provider if you are using part of your grant funding for yellow buses (not applicable for charter, private, parochial, and independent schools). See Grant Manual page 8 for instructions on cost estimations.
