MSAA Requests your Signature for the Consent and Release Form
By signing below, I agree to the following terms regarding my participation and any content I provide.
In consideration of the opportunity to participate in MSAA events and appear in MSAA materials - including MSAA's websites, blogs, social media channels, and other digital or printed media - I acknowledge that I am entering into this agreement voluntarily and with full understanding of its terms.
I hereby grant to MSAA, its subsidiaries, affiliates, licensees, contractors, agents, successors, assigns, and those acting under its permission or upon its authority, worldwide and forever:
1. An irrevocable, non-exclusive, sublicensable and transferrable license to use, reproduce, distribute, prepare derivative works of, display and perform any and all content I submit, including photographs, written content, images, and other authored works.
2. Permission to capture, use, reproduce, publish, and distribute any photographs, video footage, or digital images taken of me at the event for promotional, educational, fundraising, and communication purposes. This includes all commercial and non-commercial uses across any media now known or later developed, without restriction.
3. The irrevocable right to use, reproduce and distribute my name, image, likeness, signature, performance and voice, and to portray me and describe me.
4. If applicable, permission to fully disclose my multiple sclerosis, and to state and/or imply that I endorse MSAA.
It is within MSAA’s sole discretion to use or not use all or any portion of My Appearance and the Licensed Copyrighted Works.
The right to My Appearance and the grant of a license to the Licensed Copyrighted Works shall include, forever, worldwide and without limitation, all commercial and non-commercial media and formats now known or hereafter invented, including all digital technologies, and includes the right to create derivative works, alterations, and future versions of My Appearance and the Licensed Copyrighted Works.
My Appearance shall be a work made for hire under copyright law owned by MSAA. If My Appearance is deemed to not be a “work made for hire,” I irrevocably, forever, worldwide and without limitation or reservation assign all right, title, and interest under copyright in My Appearance to MSAA.
I retain ownership of all copyrights in the Licensed Copyrighted Works.
I hereby release and hold MSAA, its officers, directors, employees, agents, assigns, and any person or corporation acting under the permission of MSAA harmless from, and defend and indemnify them against, any claims arising in any way from My Appearance, the Licensed Copyrighted Works, or if applicable the disclosure of my multiple sclerosis whether such claims are known to me now or I become aware of them in the future, including claims based on right of publicity, misappropriation, invasion of privacy, confidentiality, false light, defamation, copyright, trademark, advertising, endorsement, or moral rights worldwide and forever.
I represent and warrant that I have obtained, own or otherwise have permission to grant all rights with respect to the Licensed Copyrighted Works, and I certify that I am of full age (18) and possess full legal capacity to execute this agreement.
