Introduction

Thank you for participating in the Conditions for Learning Survey. The information that you provide will help your school, your district, and the state in determining what can be done to support your school success and engagement in learning.

This survey consists of 36 questions that cover safety, school expectations, student relationships with other students, and adult relationships with students. There are also some questions about your characteristics, such as age and grade. All answers will remain anonymous (no one will see your answers). Your answers to these questions will not be linked to you specifically and will not be reported in a way that will make your answers available to anyone else. Your answers will be combined with other students’ answers in your school district and reported as group scores rather than as individual answers. Your district will receive a summary report in the fall of 2018 and we hope that the results will be shared with you.

Survey Instructions
Before you begin each section, read the instructions and each question carefully. In this survey, there are no right or wrong answers. However, it is important that you provide an honest answer to each question and select the response that best fits your experience. The purpose of this survey is to understand what you really think, not what you think the answer should be or the answer that would please someone else.

This survey should take 15-20 minutes to complete. Don’t spend too much time on any question. If you have difficulty answering a question in a section, go on to the next question. You can come back to a question that you didn’t answer after you have completed the other questions. If you prefer not to answer a question, don’t understand a question, or can’t answer a question truthfully, leave it blank. Please note that your teacher will not be able to answer your questions once the survey begins.
