Welcome to the National Fire Fighter Near-Miss Reporting Questionnaire.
The National Fire Fighter Near-Miss Reporting System is a free, voluntary, confidential, non-punitive and secure web-based reporting system with the goal of improving firefighter safety. It is funded by the U.S. Department of Homeland Security's Assistance to Firefighters Grant Program and is managed by the International Association of Fire Chiefs.
The information gathered in this questionnaire will provide critical and useful information on how the system is currently being used and any modifications that may be required to continue the success of this program. It is estimated that this questionnaire will take 10 minutes to complete.
