This confidential survey sets out to identify what services are currently available to young people across Victoria and where there are gaps in service provision. The survey also considers what services target the 'middle years' - children aged 8-12 years old.

This survey is targeted at any organisation, service or program that provides services to young people.

The survey should be completed by a staff member with a comprehensive understanding of the services for young people provided by your organisation and other services in your local area (eg. CEO, Director, Youth Services Manager or equivalent).

If your organisation has a number of offices/branches, please also distribute this survey to those other offices.

This survey is confidential. Only nominated VCOSS and YACVic staff will view your completed survey and no individual organisations will be identified in published reports.

All survey respondants will go into a draw to win a free registration to the 'Connect For: improved outcomes for Victoria’s vulnerable young people' conference to be held in Melbourne on 25-26 June, 2012.

This conference will be hosted by YACVic, VCOSS, the Municipal Association of Victoria, Department of Education and Early Childhood Development, Department of Human Services, Department of Health, Department of Justice and the Office of the Child Safety Commissioner and will consider future thinking around education and service delivery, early intervention, partnerships, integrated service responses and participation of young people.
