Vision Screening Outcomes: Caring 4 Eyesight Program (via the Healthy Kids Check)

 
Please be advised that the Division has not had additional funding approved to extend an incentive, thus the $30 incentive payments will be paid until the end of July 2010, but no further.

In anticipation that there may be further opportunity to apply for funding, and as we have had such wonderful results over the duration of the project, we ask that practices continue to submit their vision screening results, the results of which we will continue to feed back to practices.
