Pope Francis has called for an Extraordinary Synod on the Family to be held in Rome in October of 2014. In preparation for the synod, the Pope has sent out a survey to the bishops of the Church, requesting parish-level feedback on a range of questions from Catholics from all walks of life.
The responses will help the bishops participating in the synod to get a sense for the state of the Church on the ground level - to see how the teachings of the Church on the family are understood, how they are being received and lived out, what pastoral challenges exist, and how the Church can best respond.

on the Vatican Website. What follows is an attempt to make the questions of the survey document more accessible to all, using an adaptation of the survey designed by St. Joseph’s Parish in Seattle, with the permission of the Archdiocese.
Please take a few minutes to respond to the questions on the following pages. We ask that you respond prayerfully and honestly, with hearts open to the movement of the Spirit in your own life and in the life of the community of the Church that you know.
As you take the survey, you will be able to move back and forth between pages. However, once you submit the survey, you will not be able to edit your responses.
We are hosting this survey primarily for the parishioners of Mary, Queen of Peace, in Sammamish, WA. However, Catholics from any community and geographical area are welcome to take it.
The survey is anonymous. At the same time, you are welcome to share information about yourself on an optional information page at the end of survey, if you are so inclined.
The answers will be sent to the Archbishop of Seattle and to the Vatican. Every person’s participation is very valuable. Thank you for taking part!
