1. Information for Canadian Health Care Providers and Trainees – Validation Study

 
The Mental Health Commission of Canada invites you to participate in a survey of staff attitudes toward people with mental illnesses in health care. People have different feelings about mental illness.

The objectives of this project are to collect data to validate this questionnaire and ensure that it is reliable. The information you provide is extremely important for evaluating educational programs and measuring health care provider attitudes towards people with mental illness. The questionnaire presents you with a number of statements and asks you to indicate whether you agree or disagree. There are also personality related questions which ask you to indicate whether the statement is true about your personality or false. Please answer all questions honestly if you choose to participate.

Your participation is entirely voluntary and you will not be identified. All of your answers will remain anonymous. This means that you will not be identified in any reports. Only aggregated data will be used.

Your participation in this project is entirely voluntary. Your participation or lack of participation in this project will not affect your course work or your grades if you are student or your employment if you are an employee.

If you choose to participate, you will be asked to complete this questionnaire once.

However if you agree to complete this questionnaire twice (for test-retest reliability), we will need to track the responses over time, and link the questionnaires. In order for this, we request that you please put your e-mail address in the space provided so we can send you a reminder e-mail in one month.

If you choose to participate, and would like to be entered into a draw to win one of 20 Amazon.ca gift cards of $20, we ask that you provide your email address below. If you participate in the test-retest reliability study, your e-mail address will be entered twice into the draw.

The data from the questionnaires will be password protected and stored electronically after being imported into a data file from the questionnaire website. Questionnaire data will be destroyed after five years.

Electronic data will be kept on the computer of the Co-Investigator, Aliya Kassam (The University of Calgary). A back-up copy will be kept in the offices of the Mental Health Commission of Canada (in Calgary). Electronic data will not be identifiable. At no time will information sufficient to identify any individual be presented.
