1. Information about this Survey

 
The following survey is part of a research study designed to learn more about what types of treatment people with OCD prefer and how to improve these treatments and services. You must be 18 years of age or older to complete the survey. Your participation is voluntary; you may stop the survey at any time. The alternative is not to participate without any penalty. Participation is limited to once per person. It may take up to 30 minutes to complete all of the questions. If you are not able to finish the survey in one sitting, your responses will be automatically saved and you will be able to resume the survey by clicking on the same link you were provided. The survey is about OCD symptoms and preferences for treatment this may lead to recognition and concern about a possible need for treatment. After completing the survey, you will have the option to be redirected to a webpage that contains information about treatment resources. The survey is anonymous; no identifying information will be obtained from you, your responses will not be traceable back to you and no attempts will be made to re-contact you. There is no direct benefit to you from participation in this study. You may provide information that may contribute to better understanding treatment preferences for OCD and may lead to improved services and delivery of care.

The New York State Psychiatric Institute-Columbia University Department of Psychiatry Institutional Review Board has approved recruitment of participants for the study.
