1. Trivia Tournament Registration Form

 
The Haverhill Foundation for Excellence in Education's next Trivia Tournament will be at Maria's Restaurant on Saturday, April 28, 2012. Pre-registration is required. The deadline for registration is Tuesday, April 24. The Registration Fee is $25 per person. Check-in is at 6:30 p.m. and the tournament starts at 7:00 p.m. Please organize teams of 2 to 8 members. First Prize is $300, Second Prize is $100 and Third Prize is $50. There will be a separate $50 prize for a special Haverhill Trivia round. Free appetizers and a cash bar will be available. Donations are tax deductible.

Your entry form is NOT COMPLETE until you hit the "DONE" button at the very bottom of the page - after the Team Listing. This sends your information to HFEE. When you have completed your entry form, and clicked on "DONE", you will be returned to the HFEE web site. To pay your entry fee click on "DONATE" and you will be taken to PayPal for secure payment.

If you have formed a team, each person on your team can register and pay individually, or one person can register and pay for the whole team. List your team members, their phone numbers and email addresses. Your registration is not complete until your entry fee has been paid. The deadline for registering and paying is Tuesday, April 24.
