Provider TBI Needs Assessment Survey

 

Thank you for taking the time to complete this survey of traumatic brain injury service providers. We are using this survey to find out what services are available in Kansas for people with TBI. We will use this information, along with a survey of needs people with TBI have, to come up with a plan for meeting unmet needs with public and private resources.

It would help us the most if you answer all the questions, but most of the questions are optional. If you don't want to answer a question, just skip it.
