Everyone affected by stroke has an experience that is unique and individual to them. It is vital that every person has the opportunity to tell their story. The information you provide in the questionnaire will be fed back to your hospital with a view to improving stroke services. It is only by engaging with you that problems can be highlighted, examples of good service can be recognised, and improvements can be made.

This questionnaire gives you the opportunity to provide feedback on your personal experience of the care and treatment your relative, or the person you care for, received whilst at hospital. Subsequent questionnaire packs will look at rehabilitation.

The questionnaire will be anonymous, but we do also ask for your contact details at the for future involvement and input. You can leave this blank, but if you do feel able to give them to us, please also indicate how you would like to be involved.

Your contact details will only be held by The Stroke Association to contact you as you have requested.

The data we receive will be analysed and we will produce an annual report on our findings which will be fed back to your hospital. If you would like to receive a copy of this report, please indicate this in the contact section.
